While participating in a Conifer Canine LLC event, I understand that dog training can be both mentally and physically intensive and demanding. I understand that being around dogs and working with dogs may involve inherent risk of injury or worse. I feel as though I am in suitable physical and mental condition to attend the event in which I have applied for and understand that if I am not physically or mentally able to perform the tasks that are required to complete the course, this may affect my overall score and may hinder my ability to complete the course. Furthermore, I will hold Conifer Canine LLC not liable for any injury, or expenses as a result of injury – to myself or my dog/s – while performing tasks as a participant in a Conifer Canine LLC event.
I understand that there are no refunds for tardiness, and/or missed events, and that either will affect my score. I understand that Conifer Canine LLC and/or it’s staff reserve the right to refuse my participation at their sole discretion. I understand that during my participation a Conifer Canine LLC event that I will be held to a specific Code of Conduct and that I will be solely responsible for my actions and the actions of my dogs that of any dogs that I work with during the event. I will indemnify Conifer Canine LLC for any expenses incurred as a result of my actions or that of the dog/s I work with. I also understand that if I do not adhere to this Code of Conduct that I may be asked to leave the event. I further understand and accept that Conifer Canine LLC cannot and does not imply or guarantee my success as a participator.
I understand that Conifer Canine LLC does not express or imply a guarantee that I will approve of the event or lodging. I understand that my travel expenses to attend and/or lodge are my responsibility and at my own risk.
I understand that Conifer Canine LLC is a smoke-free, vape-free, alcohol-free, drug-free environment.
I, the undersigned, to hereby consent and agree that Conifer Canine LLC, its employees or agents have the right to take photograph, videotape, or digital recordings of me and to use it in all media exclusively for the purpose of supporting and promoting the mission of Conifer Canine LLC. I further consent that my name and identity may be released therein or descriptive text or commentary. I do hereby release to Conifer Canine LLC, its agents, and employees all right to exhibit this work in print and electronic form publicly or privately and to market my identity or likeness in whatever media used. I understand that there will be no financial or other remuneration for recording me, either for initial or subsequent transmission or playback. I further agree that such photographs or video footage may be used for educational and marketing purposes in Conifer Canine LLC promotional video materials and or third party use by funder or prospective funder. I also understand that Conifer Canine LLC is not responsible for any expense or liability incurred as a result of my participation in this recording, including medical expenses due to any sickness or injury incurred as result.
I represent that I am at least 18 years of age, that I am the above described individual, that I have read, understand, and agree to the foregoing statement/s, and am competent to execute this agreement. This consent shall remain valid until written notice is provided to the contrary.
